It was lot more difficult when I had mine done, because the rotors needed to be resurfaced.

Did you ever get the answer on what brake pads are needed?  Mine is a 2007 with 365s on the front so maybe we will have the same pads for I need new ones on my front axle and tag axle.

The slide pins are critical. One of mine was frozen and I had to replace the whole unit. You should have boots over the open end to prevent water erosion and freeaing.
