Is it Corian (Solid Surface)? Any household countertop shop should be able to make one. Working with solid surface is easy, very similar to woodworking. I don't think you could glue it without a line showing unless it is a single color. The adhesives are available for the different colors. I have worked with solid surface and have had things made out of it, the obstacle is finding a shop that had that specific color in stock. They normally order it by the sheet which would jack the price for a cover way up.
